    I have a business management degree and I'm interested in opening a homehealth care (non medical) staffing agency in the Atlanta, GA & Houston, TX area; it will be a small staffing business initially run out of my home. I have no idea where to even begin. Do I have to have a RN on my staff, and where do I go for information regarding licensing, medicaire/medicaid for the state of Georgia and/or Texas?

    @Hazem, Hello Hazem, your question is a valid one. Allow me to begin by saying that any individual or entity (corporation, LLC, DBA, etc.) who provide health care services, whether home health, clinic, adult day care, assisted living, hospital, nursing home, group home, rehab, hospice, etcetera, is titled a provider. Therefore, you can be an individual provider or an entity provider. Depending on the services you provide, you are classified into a "provider type". In the case of an individual home health aide, they can provide services such as personal care, homemaking, companion, chore, respite, etcetera. So in regards to your first question, a home health aide can be an individual provider. In regards to the 2nd question, the agency is the provider and the home health aide is the staff member contracted by the home health agency. I hope this helps clear your questions. It acts as a home health agency, however, it can only provide services to Medicaid recipients, private pay, and private insurances. The start-up costs run between $10,000-$20,000 and this includes our fee for taking you through the whole process.     @iris ramos, Hi Iris! I am always excited to hear someone in Puerto Rico is looking to start a health care business. I am Cuban with Puerto Rican roots. In regards to your question, I can tell you it is very hard to find grants in today's economy. The pointers I can give you though, is that bank of america has given money in the past to starting businesses. However, I am almost sure you will have to be a guarantor. I know SBA does not really provide much money either and in regards to grants, you have to check in your local government for those. I know at a local level, cities do offer grants as incentive to businesses, usually non-profit organizations. You will need an estimate of $60,000 to get started.     Hello Lillian, I am a registered radiation therapist for over 20 years. I will not have a job starting oct. of this year. Instead of looking for a job in my field since it is physically demanding and the pay is limited I am thinking about opening a home health care here in Michigan. My question is should I buy an existing home health already running or should I start one from scratch? Most likely I will have a partner. If I buy an existing agency then I can receive income right away right?Starting from scratch I probably will have to wait about one year to generate income. Do you also guide on who to get patient referrals from since I don't have any contacts?Thanks for your time!

    @jay, Hi Jay, how are you? I am sorry to hear about your losing your job. Both of the choices are good. The first choice of purchasing an agency though, is riskier. Here's the issue, you don't know what you are buying. Unfortunately, nowadays, you do not have to be committing fraud to be shut down. Nowadays, you can be shut down by not being compliant with rules and regs. Therefore, you have to make sure you are purchasing a good standing business. A company like ours will help you make sure the business you are buying is in good standing. Although it is never 100% sure since it all depends on the size of the company and how many years they' ve been in business, we can give you the green light to buy or red light not to buy. We will check everything, from client records to staff records, to financial documents, looking for specific items that will will tell us how the business has been managed. In addition, you will not be able to make money right away, you still need to be approved by CMS and the other regulating agencies, if any, that are involved. In other words, CHOW paperwork has be completed and sent correctly. Our agency can also take you through that process since we work with mergers and acquisitions. In option 2, yes, it can take you 9 months - 12 months, but you are starting something from the beginning in which you are aware has not committed any fraud or has started from the beginning with efficient management and has been compliant. Remember, two things can get you shut down, fraud and compliance. It's just that the risks are higher when you buy one than when you start from scratch. I have been hired in situations where companies have had their provider number terminated and we have to prepare an appeal for that. Although we have been successful in all cases, it is still not a guaanteed process. In other cases, companies have bought a business for let's say $400,000 and 30 days later have received a letter from Medicare stating they have to return over $1,000,000 for wrongful billing, and have been forced to shut down down to not having the money to return. In other cases, they have bought a business and it's turned out that the previous owner has committed fraud, and they have had to go through the whole process of proving they weren't involved in the fraud, but have been forced to shut down anyway because they lose their billing privileges. There are many factors you face, And both choices have their ups and downs. Not to mention, a lot of times, it will cost you triple the amount of money to buy an established agency than to start one, therefore, it is still cheaper for you to start it although you won't generate money right away. An established agency will cost you anywhere from $150,000 + depending on size of company and starting one should not cost you more than $60,000 throughout the whole process. Yes, part of our consulting packet does guide you through techniques of generating patient referrals.     Hi Lilian: My name is Alex and I am from New Jersey. I have a business plan, registered Elderly Care Solutions with the state of NJ and have the FEIN. I also received applications from the NJ Department of Consumer Affairs to open a Health Care Service Firm that I have not yet completed. I have written a 241 page policy and procedures manual as well as the employee manual, hundreds of different forms and the health aide manual, however, I am still very confused on the process of getting Medicare which seems to be the where most people get stalled. I am not sure if NJ has any moratorium on getting Medicaid. I have a Masters in Public Administration and for years have worked with disable individuals.     Hi Lilian, We are a group of 3 RNs trying to start a home health agency in Dallas, TX. I've read that you estimate about $60K or less for start up costs. Could you please give me a breakdown of the costs or tell me what determines those startup costs? is there a way to bring them down? Is the medicate contingency funding relative to the expected expenses or is it a minimum fixed amount? Could we start out of a home office in the state of TX? We would also work for free until the company is certified. Also, how long do you estimate the certification process to take? Thank you for your advice in advance!

    @cesar, hi and thank you for your inquiry. Since you are 3 nurses, yes the cost can be less as you can see patients yourselves and not spend on salary. We are looking at an estimate of $40,000-$50,000 contingent upon the fact that you are three nurses and you will save on staffing expenses. In Texas, depending on where you want to start it, you may be able to start from home. According to the state, they don't care if you start in a commercial address or a residential, however, at the local (city) level, it differs. Once we are hired, this is something we uncover for you. In the state of Texas, there is a licensure process as well as the certification process. Licensure can take 3-4 months and certification can take 4-6 months, in certain cases, it has taken longer than that, but it hasn't been the norm. The 60k breakdown, is basically a projection for clients who have to hire the nurses, and who have to start from a commercial address as a requirement. Once we start working together, there is a business plan provided to you as well as a breakdown of the costs to come.     Hello Lillian: I've been researching literally all night. I just happened to stumble upon this website and I must say you are amazingly knowledgeable!! I'm in oklahoma. I've researched the ok dept of human/health websites, SOS website, CMS websites, etc. I do have the jist of things however in regards to an estimated start-up costs, I have yet to figure that out. I have read all of your posts on here...just nothing for OKla. I'm not sure if Oklahoma is a need state or moratorium state. I do have a private investor as my partner and I will be attending school to become the admin to begin the business. It will be a medical home health agency specializing in behavioral illness in men. I probably know the answer to this first question, but will we still have to go through all same processes, applications, etc even if we're specializing in a particular area? Can u also give an eta on licensing, accreditation in okla? Would it be wise to join the all national assocs and get a d&b duns number? To answer your first question, yes, you would still need to undergo licensing process as a regular home health agency. In the case of Oklahoma, I do believe it is a CON state (certificate of need). I am waiting for Rod to call me back and confirm. The estimated time frame for the process is usually standard across the board, 9-12 months. The investment would be about $60,000 throughout the whole process. Yes, you will need a board of directors established and a PAC established with an MD or DO as a member. We will need to see if you qualify to be an Administrator, if not, the DON can be your Administrator and another person can be used as the Alt. Administrator. We do provide business plans for a fee.     Hi Lilian, I tried to find my question in the previous posts so that you wouldn't have to repeat it:) I live in Dallas Texas, and I need the steps to starting a home health agency. I certified as a nursing assistant. I currently work in human resources/accounts payable within an assisted living environment, and I have a degree in paralegal studies. I believe that pieces of my background can put together a well thought plan, but I need help at an VERY inexpensive rate. I spoke with a the local SCORE chapter, but my no one had attempted to open a home health agency through them in years so they were unfamiliar.

    Do I need a state license to hire employees, receive medicare payments and/or insurance payments?
    Where do I find the census for the amount of home health agencies in my area to do a competitive analysis?
    I am confused.... Will you help me?

PHC is a type of program and it takes at least one year of you being licensed as a home health in addition to providing services to10 consumers in order to contract with PHC.     P.S. There is more to the process of medical billing than what you are taught in a school setting. There is some risk involved in the process as well. It is a great career to be in, very comfortable in the sense that it is flexible so you may work from home as you look after your kids.     @Vanny, Hi Vanny! Starting a non-medical agency in Ohio, we would have to find out the type of licensing that "may" be required. You can find out with the Department of Health. In Ohio, skilled home health agencies deal directly with Medicare certification. There is no license really issued.     I have started a new minority owned personal care agency to the elderly in NYC. I submitted a request for a license over 6 months ago and was told the process can take up to 1 year! I have been set up and been advertising for 6 months, but have no clients!! Can you tell me how to best market my business to get real results? Since I do not have a license, I cannot bill for Medicaid or Medicare because I have to go through the Dept of Aging and cannot register with them until I have my license...I am in a catch 22 situation...

We are in the process of getting accredited and My partner and I was told the we would needed $100,000 in the bank the day of the inspection. Can you please let me know if that's correct because we had the understanding we needed at least 3mths expenses in the bank?     @Jacqui, Hi Jacqui! When you say, accreditation it is not the same thing as certification. Accreditation is when a company such as Joint Commission, CHAP or ACHC come out to your business and conducts a 3-day survey (for agencies that are per-existing that survey may be more days depending on the number of clients). At the end of the survey, they will award (tell Medicare) that you are in full compliance with the COPs (conditions of participation) and with their accreditation standards. For this accreditation process, you do not need $100,000. The accrediting organization does not check any monies from the agency. They check your financial documents such as your annual budget. For the certification process, yes you do need to have contingency finding money which I have stated in posts above, it would be beneficial to you if you read the discussions from earlier this year which are available above. There it explains all about the money. For Hollywood, FL, I would count on about $75,000-$80,000. But the highest I've seen in the whole country has been $98,000 and that is for the area of Miami.     @Papy... Good evening, I believe it is wiser to go ahead and franchise by using the name of your current Home Health. However, it would not be considered a parent office per say, it would be more like your Corporate Headquarters. Each office you open throughout the country, will have to follow the rules and regs of the state in which it will be located and also receive its own stand alone certifications. Meaning, you will receive new provider numbers for each location you open. This in the long run, will bring more value as a company. I live in Maryland, and i'm trying to start my own home care business, but i want to start something that will produce the money to enlarge the business itself, someone told me about starting with companion agency but i don't even know how to go about it, exactly what do i need to go about the companion agency business and if it's not too strenuous to start up my nursing agency business i wouldn't mind jumpstarting it...pls what do you suggest that i do? do have anyone in mind who can do this in maryland or can you start whatever you suggest for me? just that you know, i'm a Licensed Practical Nurse, close to obtaining my RN BSN in about a year. i have registered the business already, i have my EIN already but don't know where to go from there......pls reply asap, thanking you in advance for your prompt response.

Under State of Virginia law, what I need to have in order to operate a H/H/C agency?     @Monique, Hi Monique, definitely, a home health care agency is your most profitable choice. Medicare does not reimburse solely for home health aide services, home health aide services through Medicare is covered in combination with a skilled services. As a home care agency, you are limited in working with insurances as well as the services you will be able to provide. Now, in Texas, I can tell you that even though you offer non-skilled services, you would still hold a home health agency license, but you would be considered non-medical if what you choose to provide is solely home care (non-skilled) services.     @my duong, Good afternoon, basically it "should" be ok with the law to be hired by private patients to see them and get paid. However, the services would have to be provided by you, no hiring other individuals. Once you get into hiring other individuals, then you must register as a business and go through licensing process. That is how you begin your home health agency. Therefore, you must decide whether you wish to do this individually for yourself, or if you wish to hire more individuals and then you must form a company.     Hi Lilian have quick question. Live in NC just found out about CON. Is there anyway around this? Seems like that is more of a monopoly thing more than it is to help. We want to go forward with our agency on cash basis. As well as Medicaid. It looks like Medicare is out of the picture. What would you suggest?     Hello, My brother lives in Florida, but me and my RN sister live in CA. He would like to start a Home Health Service in CA and have me and my sister run it. My sister does not want to put money down and does not want her license number being used on the application. 1. Can we still open a business somehow? 2. If the application process takes 9-12 months and we're spending the $60K, how soon can we start accepting patients and how long is the breakeven period? I've heard that it will take a year to breakeven after you get licenses because we need to take 6 trial patients and 3 regular patients for a year. Thanks for your response.

    I am amazed at the wealth of knowledge and information that you share with people. Reading through some of this has helped to reiterate a lot of what I have already learned. Currently, I am in the early stages of my business plan. Thankfully, the Good Lord has blessed me with a the small business director at a local college that is helping me one-on-one with much of my endeavors. However, I still feel very lost in several areas. I am a Registered Nurse with business plans in the Home Care/Home Health area. I have worked in home hospice and LOVED it. For some reason, I have always been drawn to the elderly population. I thought that going back to school would help me with my business endeavors. The problem with that is that I am not sure which route to go. I want to offer the best holistic approach possible for my future clients. In your OPINION, which areas do you think would be of greater opportunity? Social Work, Health Care Management, Public Health, or continue with nursing to a Masters in Gerontology. It would be great if there was a way to get a nice splash that included all of these, but I know that is not feasible.     We fell into a very rewarding situation helping a senior citizen with 24/7 non medical care. By using 1099 contractor agreements we have 5 caregivers working "for" us. We would like to add another client or two but don't want to get into a big time business or anything medical, just activities of daily living. In Tennessee, it looks like having one client is OK, but more clients mean we would have to get licensed by the state. When would we officially be termed an agency or can we get by without the $800+ licensing? Which State Agencies in TN really care about small businesses providing personal, non-medical care for the elderly?
